The Rule Committee has approved minor amendments to the following forms for immediate use:

Forms to start a new claim

• Application for Public Sector Disciplinary Appeal (DOC, n/a) (Form 4A)

Forms to respond to a claim

• Employer's Response to Unfair Dismissal Application (DOCX, n/a) (Form 7B)
• Employer’s Response to Public Sector Disciplinary Appeal (DOC, n/a) (Form 4B)

The Notification of an Industrial Dispute (Form 4) has been updated to include additional notes designed to assist parties to identify who may notify a dispute. The header has been updated in each of the forms to reflect the Industrial Relations Commission Rules 2022.

These forms are available on the Commission's website for use from today.
